Booking Form

Welcome to Miller-Beck Institute of Music Booking Form!

How to use:

1. Click the “Select Service” drop-down: Choose the lesson you like. ie. Piano or Violin or Viola.
2. Click the “On or After” date picker: Select your preferred class date.
3. Click “With” drop-down: Automatically, all teachers teaching your chosen instrument will appear in the “With” drop-down selection. All other teachers who don’t teach your selected instrument will be hidden.
4. Once done, click the orange button “Check Availability” to check the teacher’s available time on your selected date. You may view a teacher’s availability for up to a week.
5. Select your preferred day and time and then click the orange button, “Book Appointment”.
6. An Account Info window will open to get your details (Name, Last Name, Email, Phone number, and comment). After filling out all the required information, click the gray button, “Submit” and that’s it!
   1. You will receive an automated email confirming your selected booking schedule. (Please check your Spam or Junk Folder too, if it’s not on your Inbox)
   2. If you need to cancel your booking, we provided a link found at the bottom of your booking confirmation email. Simply click on the link and your booking will be automatically cancelled.
